Registration Procedures
Students registering for a school-sponsored interscholatic team, must have a current physical examination on file. Click on the NEW Physical Form Procedures. Once the physical examination is completed, the Preparation Physical Evaluation Medical Eligibility Form should be submitted only. It will be reviewed and approved by the school's physician before the student is authorized to participate in practice/tryouts. You can register online by clicking the link above, but you must personally turn in the Preparation Physical Evaluation Medical Eligibility Form to the high school's Atheltic Department (C-128 or the Athletic Director's office).
PLEASE NOTE THAT THE PREPARTICIPATION PHYSICAL EVALUATION Medical ELIGIBlLITY FORM, must be submitted. THERE IS NO EXCEPTION. ANY FORM NOT COMPLETED WILL DELAY THE PROCESSING OF THE PAPERWORK AND THE ATHLETE BECOMING ELIGILE TO PARTICIPATE. THIS IS A STATE MANDATED REGULATION AND IT TAKES TIME TO GET THEM CLEARED BY OUR SCHOOL DOCTOR.
Note: To participate on a school-sponsored interscholastic team, each student whose physical examination was completed more than 90 days prior to the first day of official practice shall provide the health history update form (Health History Update Form) signed by the student’s parent or guardian. These forms are needed BEFORE a student is authorized to participate in practice/tryouts. Return this form to the high school's Atheltic Department (C-128 or the Athletic Director's office).
If the student-athlete has a CURRENT PHYSICAL, please drop off the PREPRATION PHYSICAL EVALUATION MEDICAL ELIGIBILITY FORM in the Athletic Trainer Room (C128) or the Athletic Director's Office. WE ARE NOT ACCEPTING THE PHSYICAL PACKET AND WE CANNOT DOWNLOAD FORMS FOR ANY REASON.
Please be aware that it takes time to process the physical exmaination paperwork.
Physicals submittted after the due date may not be cleared for the first practice.
The sports registration deadlines are listed below:
Registration Deadlines
Fall sports physicals are due on July 15, 2024
Winter sports physicals are due on October 31, 2024
Spring sports physicals are due on February 28, 2025
It is your RESPONSIBILITY to REGISTER BEFORE THE CLOSE DATE AND TO BE THERE for the tryout period.
Any athletes missing tryouts may NOT be guaranteed a tryout.

Fall sports begin: NJSIAA Fall Start Dates
Winter sports begin: NJSIAA Winter Start Dates
Spring sports begin: NJSIAA Spring Start Dates
PHYSICALS MUST BE SUBMITTED, PROCESSED AND CLEARED FOR THE ATHLETE TO TRY-OUT. PHYSICALS MUST BE VALID ON THE FIRST DAY OF PRACTICE.
If your child played another sport, contact the athletic trainer to find out what is needed (email is best or send your child to C128 after school).

The Columbia High School girls lacrosse team defeated Morristown Beard and Cranford, both at Underhill Field Complex’s Lynn V. Profeta Field in Maplewood, to improve to a 2-2 record on the season.
The Columbia High School softball team continued its unbeaten start to the season.

The Columbia High School baseball team defeated Carteret, 16-3, on Thursday, April 10, to improve to a 4-1 record on the season.

Columbia scored five runs in the fourth inning and went on to an 11-7 baseball victory over Montclair on Monday.
For the Columbia High School softball team, the past two seasons have been magnificent. This season, the Cougars are poised to make another strong run.
The Columbia High School girls basketball team featured four players who earned All–Super Essex Conference–Liberty Division honors, as selected by the divisional coaches.
