The Preparticipation Physical Evaluation Medical Eligibility Form is valid for 365 days. To participate on a school-sponsored interscholastic team, each student whose physical examination was completed more than 90 days prior to the first day of official practice shall provide the Health History Update Questionnaire Form signed by the student’s parent or guardian.  

Per the State of New Jersey and the Scholastic Student-Athlete Safety Act
With the passage of the Scholastic Student-Athlete Safety Act, New Jersey has set the standard by requiring that thorough evaluation be conducted for the silent cardiac abnormalities that may lead to the sudden death of a student-athlete. The below requirements are mandated by the state of New Jersey without exception to participate in athletics for the 2026-2027 school year.

Please Note: 

• The Medical Eligibility Form is valid for 1 year (365 days) from the date of the physical.
• New Jersey law requires all student-athletes to be screened for heart conditions that could cause sudden death.
• ALL requirements must be completed to receive medical participation clearance.
• It takes at least 2 weeks to process the physical examination paperwork, so plan ahead. 
• It is the student-athlete's responsibility to make sure their parent/guardian completes all registration steps before the deadline.
• Any student-athletes missing tryouts ARE NOT guaranteed another opportunity to tryout. 

1. Submit the  Preparticipation Physical Evaluation Medical Eligibility Form (PPE Medical Eligibility) and/or Health History Update Questionnaire form to Columbia High School Athletics Department (Athletic Department Office (inside black mailbox) or Room C-128). 

2. ANY FORM NOT COMPLETED WILL DELAY THE MEDICAL REGISTRATION PROCESS AND THE STUDENT-ATHLETE BECOMING ELIGIBLE TO PARTICIPATE.

Columbia High School Returning Student-Athlete must submit a Health History Update Questionnaire. 

Where do you submit the form? 

• Step #1: Upload onto the  FinalForms registration account. 
• Step #2:  Drop off printed copy in the mailbox located at Columbia High School in the Athletic Department Office or Room C-128. 

Columbia High School Change of Sport Form 

To become a student-manager, you must complete the registration form on FinalForms. Instead of submitting a Preparticipation Physical  Evaluation Medical Eligibility Form, student-managers must submit a Student-Manager Clearance Form.

*Student-Managers are NOT Required to submit a Preparticipation Physical  Evaluation Medical Eligibility Form, unless they plan to participate as a student-athlete in a sport during another season.
